I have a neat Challenge/Response Spam Blocking System I created. I used to get 100+ spam per day, now I get maybe 1, usually none. It's a life saver. Read all about it at:
http://scottonwriting.net/sowblog/SpamBlocker.html
Very nice.
I had thought making something like that for my hotmail account when I saw code to access hotmail in c#... but I don't get that much spam in hotmail.

Work however is Outlook/Exchange and lots of buy drugs online spam.

A couple suggestions :

Instead of a text password/kephrase, how about one of those strange image things (the ones that are not machine readable).

For the folks who re-send, on the success/confimation page, try repeating that the first email will be delivered

Couldn't the user just click a link with pre-generated enoded keys that match up to that in the DB? No need for the user to type anything at all. Spam accounts are usually fake these days, so the chance a spammer will click on (or even SEE) the link is unlikely.
